Assessment & Planning Forms

Not sure which report you should submit?  Academic Program Assessment (APA) Reports are for academic departments and programs.  All other units should submit a Support Service Assessment (SSA) Report.  If you are not sure whether you need to submit a report, check with your dean or supervisor.

For Academic Program Assessment & Planning:

Departmental Deadline:  June 12, 2026 (4:30 PM)

College Deadline:  College reports are due to Academic Affairs by July 17, 2026 (4:30 PM)
