This ECHO provides training in Social Determinants of Health at no cost, delivered right to your clinic, with a monthly schedule for your convenience. Using simple videoconferencing technology, healthcare providers connect to a community of learners which offers:

• Free Medical, Social Work, Substance Use Focused, Community Health Worker, and State Educator continuing education credits generously awarded from these organizations: MyMichigan Health, Saginaw Valley State University, Mid-State Health Network, Michigan Community Health Worker Alliance (MiCHWA), and Marquette-Alger Regional Educational Service Agency
• Opportunity to present actual patient cases, in a de-identified format, and receive specialty input
• Access to a virtual learning community for access to treatment guidelines, tools, and patient resources
