Welcome to the Cardinal Care Notes.
SVSU is a safe, friendly and respectful campus climate where we care about our students. This is demonstrated by Cardinals being noticeably connected by a sense of duty to the well-being of each other. Cardinal Care Notes provide a venue to share something from or about a student that causes you to be significantly concerned about the student's health, well-being, safety, and/or academic success. Cardinal Care Notes allow the University to offer students support and resources for social and personal issues that may impact their SVSU experience. Any member of the University community or other interested individuals may submit a concern. The information will be reviewed by the Office of Student Affairs. Thank you for caring!
Click HERE to fill out the form. If there is an immediate risk to life or property, call University Police at 911 or 989-964-4141.
Cardinal Care Notes are reviewed Monday-Friday, 8:00 am-4:30 pm when SVSU is open. If you believe your concern requires a more immediate response, please call University Police at 989-964-4141. Cardinal Care Notes are reviewed by a small group of staff who meet regularly. The information shared with this group is confidential and is used to determine the best approach to reach out to the student. Confidential Student Counseling Office information is NOT shared in meetings.
When a case is reviewed, the care team will develop an intervention plan and follow up with the individual(s) in a timely fashion.
To submit a Cardinal Care Note, please complete as much information as you are able and include any details/information you have.
