The descriptions below are intended to ensure that all health care providers submit to the Office of Accessibility Resources and Accommodations (ARA) the correct kind of information that will help the ARA staff determine which accommodations are appropriate to fit the student's needs.
The student’s current condition(s) including any relevant history.
The student’s presenting concerns (ongoing difficulties and behaviors) that may impact functioning in a postsecondary setting.
Any information that describes prior accommodations, if applicable.
Which current medications (including dosage/frequency/adverse side effects) or other treatments (psychotherapy, medication management, allergy injections, chemotherapy) are being employed.
Any additional aids required (hearing aids, assistive learning technology, visual aids, etc.).
Specific accommodations the health care provider recommends based on the how attendance at a postsecondary institution might impact the student’s condition.
The health care provider’s name and title, license number with state (if applicable), address, phone number, fax number, e-mail address and signature of health care provider.
