Step 5: Complete Benefit Forms

To review the various benefits offered including medical rates and plans, select the Support Staff Benefits link.
All benefits will be reviewed during your New Employee Orientation Meeting. Book an appointment with our Benefits Specialist, Tracie Esparza, in order to enroll. Book an appointment here: New Hire Benefit Enrollment
New employees enrolling in benefits are reminded of the necessary information for completion of the following:
- Medical Coverage – all family members (spouse and/or dependents) listed on the plans must have corresponding Social Security numbers and dates(s) of birth. Also required, a copy of the marriage license and/or birth certificates for covered dependents.
- Beneficiary information must include name, address, Social Security numbers(s) and dates(s) of birth.
Benefits will begin the 1st of the month after hire date.
PLEASE COMPLETE THE FOLLOWING FORMS
- UNUM Group Life and Supplemental Life Enrollment/Change Form (427KB) - Complete this form to enroll in group life or supplemental life insurance.
- SVSU Retirement Plan (94KB) - Review the TIAA – Retirement Plan and enrollment instructions
- 403(b) Compensation Plan Agreement Form (874KB) - Complete this form to authorize salary reductions for 403(b) retirement plans.
- 457(b) Deferred Compensation Plan Agreement Form (936KB) - Complete this form to authorize salary deferral for 457(b) retirement plans.
- Flexible Spending Account Overview (33KB) - If enrolling in Flexible Spending Accounts, determine annual contributions for dependent/child care and/or health care accounts.
