Student Employee Application (46kB)  Complete the Student Employment Application.
Student Employee Direct Deposit Form (91kB) Student Employees may elect to have their pay deposited into a financial institution of their choice via the Student Employee Direct Deposit Form.

 

Payroll & Tax Forms

I-9 Form (551kB)

Employment Eligibility Verification – Federal law requires all employees to provide verification that they are eligible to work in the United States.

You will be asked to provide two or more forms of original identification. Either one (1) document from List  A OR one (1) document each from List B and List C as outlined on page 9 of the I-9 Form.

The most common forms of documentation are noted below:

-Permanent Resident Card or Alien Registration Receipt Card
   (Form I-551)
-An un-expired foreign passport with a temporary I-551 stamp
-Driver's License   -Social Security Card
-Birth Certificate

 

W-4 Federal Tax Form (113kB) Complete the Federal Income Tax Withholding Certificate.
W-4 Michigan State Tax Form (37kB) Complete the Michigan Withholding Exemption Certificate.
SW-4 Saginaw City Tax Form (35kB) If Applicable, complete the City of Saginaw Withholding Certificate (CITY OF SAGINAW RESIDENTS ONLY). 

 

Family Educational Rights and Privacy Act of 1974 (FERPA)

All SVSU employees are required to comply with the Family Educational Rights and Privacy Act of 1974 (FERPA).  The University must provide each employee with a copy of this policy and obtain written acknowledgment of receipt by signing the Acceptance of Access and Responsibility Form.
