Step 2: Complete Payroll/Tax Forms

Note: Paperwork will not be processed until the successful completion of your background check. 

Complete all forms in this section and submit them to the Human Resources Office on or before your first day of employment.

Employee Data Form

  • Employee Data Form (56KB) - Complete the data form which provides the University with basic employee information.

Payroll & Tax Forms

Federal law requires all employees to verify they are eligible to work in the United States.

You will be asked to provide original identifying documentation in person. Acceptable forms of identification are outlined on page 9 of the I-9 Form. Human resources will make a copy of the document(s) below.

Please bring either:

1 document from List A 


1 document from both List B and List C (2 documents total)


The most common forms of documentation are noted below: 





-U.S. Passport (unexpired)
-Permanent Resident Card or Alien Registration Receipt Card
   (Form I-551)
-An unexpired foreign passport with a temporary I-551 stamp
-Driver's License   -Social Security Card
-Birth Certificate


Family Educational Rights and Privacy Act of 1974 (FERPA)

All SVSU employees are required to comply with the Family Educational Rights and Privacy Act of 1974 (FERPA).  The University must provide each employee with a copy of this policy and obtain written acknowledgment of receipt by signing the Acceptance of Access and Responsibility Form.

  • FERPA Staff Form (171KB) - Administrative Professionals, Support Staff & University Police review and sign the attached policy.
  • FERPA Faculty Form (172KB) - Faculty & Adjunct Faculty review and sign the attached policy.
