Getting Paid
There are several forms/documents that you must complete upon employment at the University in order to get paid. There is no need to wait for your first day on the job to process these forms. Feel free to get a head start today by clicking on the links for the individual forms and documents under the related headings listed below. Please do not delay in completing this paperwork. Your pay and benefits cannot be processed without these important documents.
I-9 Form (Reqired form - Verifies that individuals are eligible to work in the United States)
- Federal law requires all employees must provide verification that they are eligible in the United States. This is done by bringing the I-9 form with you on your first day and completing in person with an Human Resources representative within three days of hire. You will be asked to provide two or more forms of identification. Either one document from List A, or one each from List B and List C as outlined on page 4 of the I-9 Form. The most common forms of documentation are:

- Permanent Resident Card or Alien Registration Receipt Card (Form I-551)
- An unexpired foreign pasport with a temporary I-551 stamp
- Driver's License
- Social Security Card
- Birth Certificate
- Federal W-4 (Federal Income Tax Withholding Certificate)
- MI-W4 (Michigan Withholding Exemption Certificate)
- SW-4 (City of Saginaw Income Tax Withholding Certificate) This form is only completed if you are a resident of the City of Saginaw.

- FERPA (Faculty)
SVSU faculty is required to comply with the Family Educational Rights and Privacy Act of 1974 (FERPA). The university must provide each faculty member with a copy of this policy and obtain written acknowledgement of receipt by signing the Acceptance if Access and Responsibility Form. Student information through the Cardinal Direct and Colleague systems will not be accessible until this document is signed. - FERPA (Staff)

You can elect to have your pay deposited into the bank account(s) of your choice via the Direct Deposit Enrollment Form.
Completed forms can be faxed to Human Resources at 989-964-7066, mailed to Saginaw Valley State University, 7400 Bay Rd., University Center, MI 48710, Attention Human Resources, or dropped off at the Human Resources Office, Wickes Hall, Room 373. New Employee Orientation - Information and Registration
The New Employee Orientation, conducted by Human Resources, provides a comprehensive introduction to employment at SVSU, including employee benefits.
How to sign up for Orientation
New faculty, administrative and professional, support staff, police and facilities employees should call 989-964-7100 or e-mail hr(at)svsu.edu. New employees should bring the following information to the session:
- Information about family members to be included in insurance coverage (spouse/dependents), including name, Social Security number, and date of birth
- Beneficiary information including name, address, Social Security number, and date of birth
- Personal Identification for completing the I-9 Form
- A ballpoint pen
