Procedures for Requesting Services

  1. Students must self-identify and provide adequate documentation of the disability. After reviewing the documentation, the coordinator will discuss with the student for which services she or he is eligible.
  2. Students must request services for each class each semester. Students must submit an official class schedule each semester requesting services. If using testing room students also must provide their course syllabi.
  3. Requests for regular classroom accommodations must be made three weeks in advance. Requests for Braille, sign language interpreters, and other services contracted out should be made at least six weeks in advance.
  4. The Office of Disability Services will notify faculty in writing for which accommodations the student is eligible to receive. The student will discuss with faculty how those accommodations can best be implemented.
  5. Individuals may file complaints on matters related to disabilities with the ADA advisory Council. Initial contact should be made with the office of Disability Services.
