Cooperating Teacher
University Supervisor
The two-part Midterm Evaluation is completed and signed by the Cooperating Teacher at the mid point of the student teaching experience. This is completed in conjunction with the Disposition and Evaluation Rubrics. The Midterm Evaluation can be completed on-line and printed or hand written as this document does not become part of the student’s permanent record. (The Cooperating Teacher may wish to make a copy of this evaluation as it is similar to the Final Evaluation.) The Student Teacher adds comments, also signs the Midterm Evaluation, and is responsible for getting the Midterm Evaluation to the University Supervisor in the requested time period.
The two-part Final Evaluation is completed on-line and printed by the Cooperating Teacher and by the University Supervisor near the completion of the student teaching experience. The Final Evaluation (completed by the Cooperating Teacher) is signed by the Cooperating Teacher and the Student Teacher. Unless otherwise designated by the University Supervisor, the Student Teacher is responsible for returning the completed and signed Final Evaluation to the University Supervisor. Copies of all evaluations may be made by the Student Teacher for his/her files.
Students will be completing three surveys at the end of their student teaching experience. The web addresses to these surveys will be distributed at the last seminar. We will also email the correct web addresses to each student's SVSU email account after the last seminar. The three surveys are:
1. Michigan Department of Education Survey - This is an anonymous survey required of all student teachers in the state completing their Teacher Education program.
2. College of Education Survey - You will be self evaluating yourself using the Disposition and Evaluation Rubrics that you have used throughout your education experience.
3. College of Education Anonymous Survey - You will be answering questions regarding your SVSU Supervisor, host school, Cooperating Teacher and seminars.
