The Disposition Rubric and Evaluation Rubrics are used in conjunction with the Student Teacher Evaluations:

• Two-part Midterm Evaluation. For the Midterm Evaluation, the Cooperating Teacher may complete it on-line and print it. Or this evaluation may be hand written as this document does not become part of the student’s permanent record.
• Two-part Final Evaluation. The Final evaluation is completed by the Cooperating Teacher and University Supervisor on-line and printed.
• Supervisor Visit Form (completed by the University Supervisor)
• Student Teacher Self-evaluation (completed by the Student Teacher).
